Primary duties of the Psychologist are to provide Integrated Mental Health (IMH) services using the Primary Care Mental Health Integration (PCMHI) model to multiple VISN 21 facilities via the use of telehealth as part of Clinical Resource Hub (CRH), primary care (PACT) section. While the VISN21 CRH PACT PCHMI Psychologist will be administratively connected with the SFVAHCS, the physical location is negotiable and will be largely virtual. Services will be provided VISN-wide in close coordination with other CRH team members. There may be requirements to be on-site at Ft. Miley on a quarterly basis for staff meetings.The integrated mental health (PCMHI) team works as part of a telehealth primary care team (a clerical associate, provider, pharmacist) together with patient site care team members (a clerical associate, clinical associate, and RN care manager) to provide telehealth primary care to Veterans. The incumbent will provide services to primary care patients in a collaborative framework within primary care teams. Behavioral health visits are generally brief (20-30 minutes), limited in number (1-6 visits), and are provided in the primary care practice area structured so that the medical providers are supported across a broad scope of behavioral health concerns. The incumbent's role is structured so that the patient views meeting with the behavioral health provider as a routine primary care service. The incumbent will utilize mechanisms for systematic monitoring of symptoms and treat emergent problems such as non-adherence or side effects; decision support; patient education and activation; and assistance in referral to specialty mental health care programs, when needed. Clinician will utilize brief interventions that are clinically appropriate, based on patient need to assist Veteran with depression, anxiety, stress, sleep hygiene, and other areas of behavioral health medicine. Treatment could include individual or group psychotherapy, brief interventions, and evidence based therapy. Clinical Responsibilities (90% of time). Via warm handoff or referral, provides functional assessment and a full range of psychological interventions consistent with training and expertise, with special emphasis on brief and evidenced-based interventions found to be effective in primary care settings. Works toward department expectation of 4 clinical encounters daily. Facilitates patient and family education appropriate to the primary care setting and the specific presenting mental or behavioral health issues. Provides consultation services for telehealth primary care providers and other PACT staff to maximize the effectiveness of their interventions in reducing health-risk behaviors and enhancing health-promoting behaviors, with a significant emphasis on a prevention-oriented model of care. Provides a full range of therapeutic interventions including:
individual and group therapies for a full range of behavioral, adjustment and lifestyle problems. Follows PCMHI model of care for brief, targeted therapy sessions (e.g., 1-6 sessions) and facilitates referrals indicated for longer term or more intensive treatment. Completes required clinical and administrative documentation in a timely manner and in accord with governing regulations. Teaching/Training:
( =5% of time) May be involved in the training of psychology interns, psychology post-docs, medical students, medical and psychiatric residents, nursing students, fellows and other graduate or post-graduate trainees, as appropriate. Provides staff and student didactic training and consultation as requested in content areas of scholarly and/or clinical expertise. Serves as a consultant to other VISN 21 medical center staff and trainees and assists in their in their formulation of the psychological characteristics and appropriate treatment expectations regarding their patients. May be involved in training other VISN 21 facilities in PACT and/or telePACT. Research/Program Evaluation/Program Development:
(=5% of time) May participate in program evaluation and/or research activities. May assist with, develop, and/or facilitate development and implementation of VISN 21 Virtual IMPACT HUB and site specific IMH programming. Administration:
(=5% of time) May participate in program evaluation and/or research activities. May assist with, develop, and/or facilitate development and implementation of VISN 21 Virtual IMPACT HUB and site specific IMH programming. Participates as a voting member of the medical staff of the San Francisco VAHCS. Serves on Medical Center or Healthcare System, VISN, and/or National VA committees or task forces, as endorsed by supervisors. Physical Demands of the work are generally minor. The work is primarily sedentary, although the stress involved in dealing continually with persons in emotional distress and who may be uncooperative and occasionally violent makes this work notably more demanding than most sedentary jobs. (1) Have a doctoral degree in psychology from a graduate program in psychology accredited by the American Psychological Association (APA), the Psychological Clinical Science Accreditation System (PCSAS), or the Canadian Psychological Association (CPA) at the time the program was completed. The specialty area of the degree must be consistent with the assignment for which the applicant is to be employed. For the purpose of meeting this requirement, the term specialty area refers to the specific specialty areas recognized by the accrediting body and not to specific job duties that might require special skills. Currently, APA accredits doctoral programs in the specialty areas of clinical psychology, counseling psychology, school psychology, or combinations of two or more of those areas. PCSAS accredits doctoral programs in psychological clinical science. CPA accredits doctoral programs in clinical psychology, counseling psychology, clinical neuropsychology, and school psychology. There are no job assignments in VHA that require the skills of a school psychologist; therefore, an applicant with a degree in the specialty area of school psychology is not eligible for appointment. Strictly for the purpose of determining eligibility for appointment as a psychologist in VHA, there is no distinction between the specialty areas (with the exception of school psychology). OR (2) Have a doctoral degree in any area of psychology and, in addition, successfully complete a re-specialization program (including documentation of an approved internship completed as part of the re-specialization program) meeting both of the following conditions:
(a) The re-specialization program must be completed in an APA or a CPA accredited doctoral program; and, (b) the specialty in which the applicant is retrained must be consistent with the assignment for which the applicant is to be employed. NOTE:
The applicant must provide an official transcript and/or certificate documenting the completion of the re-specialization program, which includes completion of an APA or CPA internship. Psychologists who have successfully completed a re-specialization program as described above and who were employed by the Department of Veterans Affairs prior to the implementation of this standard are considered to have fully met the educational requirements of these qualification standards. OR (3) Have a doctoral degree awarded between 1951 and 1978 from a regionally-accredited institution, with a dissertation primarily psychological in nature. AND (4) Internships, (a) Have successfully completed a professional psychology internship training program that was accredited by APA or CPA at the time the program was completed and that is consistent with the assignment for which the applicant is to be employed. OR (b) New VHA psychology internship programs that are in the process of applying for APA accreditation are acceptable in fulfillment of the internship requirement, provided that such programs were sanctioned by the VHA Central Office Program Director for Psychology and the VHA Office of Academic Affiliations at the time that the individual was an intern; OR (c) VHA facilities that offered full-time, one-year pre-doctoral internships prior to PL 96-151 (pre-1979) are considered to be acceptable in fulfillment of the internship requirement; OR (d) Applicants who completed an internship that was not accredited by APA or CPA at the time the program was completed may be considered eligible for hire only if they are currently board certified by the American Board of Professional Psychology in a specialty area that is consistent with the assignment for which the applicant is to be employed. (NOTE:
Once board certified, the employee is required to maintain board certification.) OR (e) Applicants who have a doctoral degree awarded between 1951 and 1978 from a regionally-accredited institution with a dissertation primarily psychological in nature may fulfill this internship requirement by having the equivalent of a one-year supervised internship experience in a site specifically acceptable to the candidate's doctoral program. If the internship experience is not noted on the applicant's official transcript, the applicant must provide a statement from the doctoral program verifying that the equivalent of a one-year supervised internship experience was completed in a site acceptable to the doctoral program. At least one year of experience as a professional psychologist equivalent to the next lower grade level (GS-11). Psychologists who are not licensed must practice under the supervision of a licensed psychologist but with less intense supervision than at the GS-11 grade level. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
